Look up the person behind the care plan.
Use Florida’s public records to check the person treating you. Checking that person’s permission to provide care is separate from checking the clinic business.
A licence is permission to work in a health profession. A clinician is the health professional who will check your health and plan your care. Start with that person’s full name and profession. Check the business name separately.
Discipline means the group that grants licences took action about how a person did their work.

Read the result, then ask about it
Check that the name and profession match the person who will treat you. Use the licence number to tell apart people with similar names. Look at the licence’s current status and any limits on the work the person may do.
Ask: “Does this record mean you are allowed to give the care we discussed?” If an entry is unclear, ask the health department to explain it.
